Make it mandatory yes or not have to take a stand right away or you don't know right away. The risk is that whoever fills out the form will assume yes or no... and it will be taken at face value.
the advantage of “don't know” is to show that the answer is still to be found...

For baptism (yes/no, date and place), for confirmation (yes/no).
-I would not make Communion information mandatory: people most often ignore the details (already only for baptism...) and this therefore risks blocking them in the form (this is certainly important pastoral information but not required canonically).
- For confirmation, yes/no would be sufficient at the beginning, since we will get the information via the baptism certificate.
- On the other hand, for the baptism you need as much information as possible to search for the certificate.
